Abstract
The 2009 Uganda Malaria Indicator Survey (UMIS) was implemented by the Uganda Bureau of Statistics (UBOS), the Uganda Malaria Surveillance Project (UMSP) Molecular Laboratory at Mulago Hospital, and the National Malaria Control Programme (NMCP) of the Ministry of Health, with technical direction from ICF Macro and funding from the President's Malaria Initiative (PMI) through USAID. It is the first nationally representative malaria survey conducted in Uganda, establishing a baseline against which subsequent UMIS rounds (2014-15, 2018-19) and related Uganda Demographic and Health Surveys could measure progress. The survey collected national and regional data on household and women's characteristics, ownership and use of mosquito nets (including long-lasting insecticidal nets, LLINs), indoor residual spraying (IRS), antenatal care and prophylactic antimalarial use during pregnancy (including early Intermittent Preventive Treatment, IPT), care-seeking and treatment practices for febrile children, community medicine distribution, and malaria knowledge and exposure to malaria messaging. Children aged 0-59 months were tested for anaemia and malaria infection via blood smear microscopy, with species identification. The survey was conducted against the backdrop of Uganda's Abuja 2010 malaria targets and aimed to provide the Ministry of Health, NMCP, and other stakeholders with a comprehensive biological and behavioural evidence base for designing and targeting future malaria control interventions.
Unit of Analysis
Households and individuals. Household-level data cover dwelling characteristics, durable goods, and mosquito net ownership and use. Individual-level data cover women age 15-49 (reproductive history, antenatal/preventive malaria care, malaria knowledge, and care-seeking for children's fever) and children age 0-59 months (anaemia and malaria biomarker testing via blood smear microscopy).

Coverage

Geographic Coverage
Uganda, national coverage, disaggregated by urban/rural residence and 10 survey regions (North East, Mid Northern, West Nile, Mid Western, South Western, Mid-Eastern, Central 1, Central 2, East Central, and Kampala as a standalone region). No additional sub-regional study domains in this baseline round (introduced in subsequent 2014-15 and 2018-19 rounds). Sample allocated equally across regions (17 clusters each) rather than proportional to population, so results require weighting for national representativeness.
Universe
Household population of Uganda, disaggregated by (a) all households, (b) women age 15-49 who were permanent residents or overnight visitors in the selected households, and (c) children age 0-59 months listed in the selected households and eligible for anaemia and malaria testing. Sampled from the 2002 Population Census enumeration area frame. This is the first (baseline) round of the Uganda Malaria Indicator Survey series; no refugee-settlement or malaria-endemicity study domains were included (these were introduced in the 2014-15 and 2018-19 rounds respectively).

Sampling

Sampling Procedure
Stratified, two-stage cluster (probability proportional to size) sample, not self-weighting. Stage 1: 170 enumeration areas (clusters) selected from the 2002 Population Census frame (26 urban, 144 rural), allocated equally across the 10 survey regions (17 clusters per region) rather than proportional to population. Stage 2: 28 households selected per cluster by systematic sampling following a household listing operation. Target sample: 4,760 households (4,536 occupied; 4,428 interviewed, 98% response rate). Sampling weights applied to correct for the equal-allocation design and bring estimates into national proportion. Full sampling error and allocation details in Appendix A and B.
Weighting
The 2009 UMIS sample was not self-weighting, as sample allocation was equal across the 10 regions (17 clusters per region) rather than proportional to population; a final weighting adjustment was required for national and domain-level estimates. The design weight was derived from the product of first-stage (cluster) and second-stage (household) selection probabilities — P1i = (b·mi)/Smi and P2i = c/Li (c=28 households per cluster) — giving overall probability fi = P1i·P2i and design weight 1/fi. The published report does not detail further nonresponse-adjustment or normalisation steps beyond the design weight; analysts should consult the dataset's weight variable documentation for the final applied weight formula.

Data Collection

Data Collection Mode
Face-to-face interviews using paper-based (PAPI) Household and Woman's Questionnaires, adapted from Roll Back Malaria/DHS model MIS instruments and translated into six local languages (Ateso-Karamojong, Luganda, Lugbara, Luo, Runyankore-Rukiga, Runyoro-Rutoro). No separate Biomarker Questionnaire in this round (introduced in 2018-19); consent and biomarker results were recorded on the Household Questionnaire. Biomarker data collected via finger-prick blood samples: malaria tested by Paracheck rapid diagnostic test in the field, with thick/thin blood smears sent to the UMSP Molecular Research Laboratory (Mulago Hospital) for microscopy and species determination; anaemia tested on-site via HemoCue equipment. Completed paper questionnaires were returned to UBOS headquarters for manual double-entry data processing using CSPro.
