MALARIA INDICATOR SURVEY 2014/2015

Abstract
The 2014-15 Uganda Malaria Indicator Survey (UMIS) was implemented by the Uganda Bureau of Statistics (UBOS) and the National Malaria Control Programme (NMCP) of the Ministry of Health, with technical assistance from ICF International through The DHS Program, and funding from USAID/PMI and UK DFID. It is the second national malaria indicator survey in Uganda, following the first round in 2009, and was conducted shortly after Uganda's first nationwide long-lasting insecticidal net (LLIN) mass distribution campaign in 2013-14. The survey collected national and regional data on key malaria indicators, including ownership and use of mosquito nets, indoor residual spraying (IRS) coverage, intermittent preventive treatment of malaria in pregnancy (IPTp), care-seeking and treatment practices for febrile children, use of artemisinin combination therapy (ACT) and rapid diagnostic tests (RDTs), and malaria knowledge, attitudes, and behaviour. Children aged 0-59 months were tested for anaemia and malaria infection using RDTs and microscopy. The survey provides evidence to track progress under Uganda's Malaria Reduction Strategy, which targets a pre-elimination phase by 2020, and to guide the National Malaria Control Programme and stakeholders in designing and targeting future interventions, including planned scale-up of IRS to 50 high-burden districts.
Unit of Analysis
Households and individuals. Household-level data cover housing characteristics, wealth/assets, and mosquito net ownership and use. Individual-level data cover women age 15-49 (reproductive history, IPTp use, malaria knowledge and treatment-seeking for children) and children age 0-59 months (anaemia and malaria biomarker testing via RDT, HemoCue, and laboratory microscopy).

Coverage

Geographic Coverage
Uganda, national coverage (241,039 km², 112 districts at time of survey), disaggregated by urban/rural residence and 10 survey regions (Central 1, Central 2, East Central, Kampala, Mid-North, Mid-Western, Mid-Eastern, North East, South Western, West Nile), with a combined 'Greater Kampala' zone (Kampala, urban Wakiso, urban Mukono). District-level identifiers are retained for three malaria-endemicity study domains: 10 current IRS districts, 14 future IRS districts, and 10 high-altitude (low-burden) districts. Sampling frame excludes nomadic and institutional populations (hotels, barracks, prisons).
Universe
Household population of Uganda, excluding nomadic and institutional populations (hotels, barracks, prisons), disaggregated by (a) all households, (b) women age 15-49 who were usual residents or overnight visitors in the selected households, and (c) children age 0-59 months who were usual residents or overnight visitors in the selected households and whose parent/guardian consented to anaemia and malaria testing. Sampled from the preparatory frame for the August 2014 Uganda Population and Housing Census. No refugee-settlement domain was included in this survey round (introduced only in the 2018-19 UMIS).

Sampling

Sampling Procedure
Stratified, two-stage cluster (probability proportional to size) sample, not self-weighting. Stage 1: 210 enumeration areas (clusters) selected from 20 strata drawn from the August 2014 Population and Housing Census preparatory frame (44 urban, 166 rural; Karamoja oversampled for DHS-region comparability). Stage 2: 28 households selected per cluster by equal-probability systematic sampling following a household listing and mapping exercise (Oct–Nov 2014). Target/achieved sample: 5,802 households. Sampling weights applied to correct for unequal selection probabilities across the 10 regions and 3 malaria-endemicity study domains (10 current IRS, 14 future IRS, 10 high-altitude districts). Full sampling error and allocation details in Appendix A and B.
Weighting
The 2014-15 UMIS sample was not self-weighting due to non-proportional allocation across the 10 regions and 3 malaria-endemicity study domains, and oversampling of Karamoja; sampling weights are required for all analyses. Weights were derived from the product of first-stage (cluster) and second-stage (household) selection probabilities — P1hi = (ah·Mhi)/SMhi and P2hi = ghi/Lhi (ghi=28 for all clusters), giving overall probability Phi = P1hi·P2hi and design weight Whi = 1/Phi — then separately adjusted for household nonresponse, individual (women's) nonresponse, and child-testing nonresponse, and normalised at the national level so weighted cases equal unweighted cases for each respondent type. Weights are valid for proportions, means, ratios, and rates, but not for population totals, and are explicitly not valid for pooling data across different survey rounds. Full derivation in Appendix A.4 of the survey final report.

Data Collection

Data Collection Mode
Face-to-face interviews using paper-based (PAPI) Household and Woman's Questionnaires, adapted from Roll Back Malaria/MERG core model MIS instruments and translated into six local languages (Ateso/Karamajong, Luganda, Lugbara, Luo, Runyankole/Rukiga, Runyoro/Rutoro). No separate Biomarker Questionnaire in this round (introduced in 2018-19); biomarker consent and results were captured within the household/woman's data flow. Biomarker data collected via finger-/heel-prick blood samples: malaria tested by RDT in the field, with thick/thin blood smears sent to the UMSP Molecular Laboratory (Mulago Hospital) for microscopy and species determination; anaemia tested on-site via HemoCue portable analyser. Completed paper questionnaires were returned to UBOS headquarters for manual data entry and editing using CSPro.
